If you'd like to sign your child up join us on a Monday at our large central klub, you can find a QR code to our consent form below.

Please note that there may currently be waiting lists for other activities like our Hub Klubs. If you would like to find out more about these please call us on 0113 2456533.

As you sign the consent form below please be aware of the below information:

1. It is your responsibility to make Kidz Klub aware of any medical conditions your child may have and keep this information up to date.
2. It is your responsibility to ensure that your children get to and from the bus stop or other Kidz Klub venue safely. We are only responsible for your child once they are on the Kidz Klub bus or venue.
3. It is your responsibility to make Kidz Klub aware of any changes in your details - especially if your phone number changes.
4. Unless your child is in year 5 or 6 (and we have consent) parents/guardians (over the age of 16) are ALWAYS expected to drop off and pick up their child from a Kidz Klub session/bus stop. If you are not there at the correct bus time, we will have to take them back to klub and you will need to pick them up later.
5. It is essential that your child understands the importance of good behaviour. If the team feels they are behaving inappropriately, we may need to contact you to bring them home or one of the team may bring them home.

Please Note:

• All Kidz Klub workers are DBS checked, referenced and trained.
• Kidz Klub have policies and procedures in place to ensure the safe running of all activities.
• If parents or carers wish to discuss any of these please contact us. We take seriously the care of the children who come to our activities.
• Once your child is signed up, we will deliver a Kidz Klub colouring sheet to your house each week before a Kidz Klub session. This will either be delivered in person by 2 of our team or sent in the post.
• We are following the National Youth Agency secure Guidelines and all our activities are risk assessed for safety.
